vscode: Duplicate app icons in app switcher on mac

Mac OSX 10.15.3
Since the last update, when I use “command + tab” to switch applications, VS Code shows up twice in the list. Quitting one of them quits both. One of the icons can be clicked, but it does not open the application. So it’s as though there’s one correct icon and one decoy.

This has been happening to me for the last few versions as well! When I force-quit the extra icon a message says “Extension host terminated unexpectedly”.
Used extension bisect to track the issue down to ban.spellright:
